Please read before completing application

• All applicants must be between the ages of 10 and 25.
• All applicants must be at least 6 months free of all chemotherapy treatments, unless otherwise explained and approved by a physician
• All applicants under the age of 18 must travel with a parent or legal gaurdian
• All three forms must be completely filled out and emailed to info@cwfund.org before consideration. 
• Applications will be reviewed quarterly on a first come basis.  A confirmation email will be sent as soon as your application is recieved.  If any part of the application is incomplete, we will notify you via email to assist you in properly completing the application process.
