How To Apply
Are you between the ages of 10 and 25? Did the ‘beautiful game’ help you through a cancer diagnosis and treatment? If so, you could be eligible for selection as a Craig Willinger Fund (CWFund) honoree.
Application Guidelines
- All applicants must be soccer fans and have a story to tell of how soccer helped them overcome a cancer diagnosis and treatment.
- All applicants must submit a completed application consisting of the general honoree application, medical eligibility form, and general release agreement. Only completed applications will be considered.
- All applicants must be declared fit to travel by the physician overseeing their care.
- It is the responsibility of the honoree and guest to obtain a valid passport, visa and any other documents needed for travel at their own expense and in a timely manner. Please note: travel to a foreign country by a minor child requires documented approval to travel by both parents, whether custodial or noncustodial.
If you qualify, please fill out the form below to request an application.
Applications will be reviewed quarterly on a first-come, first-served basis. A confirmation email will be sent as soon as your application is received. If any part of the application is incomplete, we will notify you via email to assist you in properly completing the application process.
The Craig Willinger Fund provides trips based on availability of funds, number of current applications, and other given variables. All applications are reviewed by the CWFund board. Applicants and their respective trips are collectively decided by the selection board. Not all applicants will receive a trip.
If selected, honorees are provided a once-in-a-lifetime soccer trip that includes airfare for an honoree and guest (parent if the honoree is under 18), double-occupancy hotel accommodations, game tickets, and a bit of spending money.
